The end of the pipe was not secured, and when the cut was nearly completed, the far end of the pipe swung down and hit the ladder out from underneath the employee. He fell to the floor and landed on his head, suffering a head injury, scratches on his back, and a bloody nose. He was hospitalized.

An employee was standing on a lift wearing his harness to remove a section of duct work. He took the bolts off from around the section of duct work and then lowered the lift. The section of duct work was supposed to lower with the lift, but when he looked back, he noticed that the duct work was not coming down. At this time, he used his upper body, shoulder area to loosen the section. The duct work then fell and pinned him underneath it. He suffered a lumbar fracture.
